DOCOH HOLD HARMLESS DISCLAIMER
In case of accident or injury to my dog or myself, including dog bites, I hold the Dog Obedience Club of Hollywood, Inc., its Instructors, Officers and Board of Directors, harmless from any claim or liability resulting from said accident or injury. I understand that animals, including my own dog (s), are unpredictable and that dog sports by nature are not without risk.
REGISTRATION CANCELLATION
If a registration is withdrawn, the class fee will only be refunded if notice is received via e-mail no less than 7 days prior to the start date of a class.
REFUND POLICY
Refunds for registration cancellations, registrations for a class already designated as full or for a class that a student or dog is not eligible for will be subject to a $5 cancellation fee.
CLASS CANCELLATION
In the event that the Club has to cancel a class or event, an alternate may be offered to the registrant. If the alternate is not acceptable or if no alternate is available, then the Club will waive the $5 cancellation fee and return the full amount of the class fee paid.
CLASS POLICIES
Please bring treats, a poop bag, a collar with 4- to 6-foot leash, water and a bowl. Bring vaccination records to the first night of class. Please wear comfortable appropriate outdoor footwear for training on pavement or clay surface. Classes on any night will not be cancelled unless there is a severe weather alert; if it is raining, classes will be moved from out in the open to underneath the Arena covering. Classes on any night may be cancelled from time to time as directed by the Town of Davie; if that occurs, your class schedule will be adjusted accordingly.
REGISTRATION ACKNOWLEDGEMENT
You will receive an auto-generated acknowledgement of this registration. The Instructor will send out a group Welcome e-mail a day or two before class starts with details about the class, where to park, where to meet, etc.
COVID-19 WAIVER
I attest to the best of my knowledge that I, and any of my guests that may accompany me to class, do not have COVID-19 at the time of attending a class and have not exhibited any signs or symptoms of COVID-19 in the 7 days preceding attendance of a class. To my knowledge, I confirm that I have not been in contact with or exposed to any known carrier of COVID-19 within the past 7 days of attending a class. I understand and agree that if, at any time while taking a training class offered by DOCOH or within 7 days of taking any class, I test positive for COVID-19, I shall contact my Class instructor immediately to inform him or her of my positive status. I further acknowledge and agree that: (i) I am attending or participating in a class hosted by the Dog Obedience Club of Hollywood at my own risk and take full responsibility for my health and safety at all times; and, (ii) DOCOH, its Officers, Board of Directors, Instructors and any other Club volunteers (collectively, the “Club Representatives”) are in no way liable for any COVID-19 exposure that may occur while I am attending any class.
ONLINE PAYMENT POLICY
After you check “Yes” below to submit this registration form, you will be taken to our payment page where payment is made via PayPal. You do not need a PayPal account and can pay as a guest using any credit or debit card. A spot in class is not secured until DOCOH receives an electronic registration and your full and correct payment has been processed. If payment is not submitted at the time of this registration, then this class registration may be voided with no further action required by the Dog Obedience Club of Hollywood.
